In healthcare, the gap between identifying what needs to be done and actually doing it is where critical information gets lost. The Care Loop closes that gap, ensuring that every action item becomes a trackable task, and every completed task is logged back to the main feed.

When an encounter generates action items—like “Increase Lisinopril to 10mg” or “Schedule follow-up in 2 weeks”—these don’t just remain as notes in the encounter record. They automatically become tasks in your task list.

This transformation is crucial because:

  • Tasks are visible and actionable
  • Tasks can be assigned to specific caregivers
  • Tasks have due dates and reminders
  • Tasks can be prioritized

When you check off a task as complete, the system doesn’t just mark it done. It logs the completion back to the main Hearth feed, creating a complete audit trail.

For example:

  • Encounter → “Increase Lisinopril to 10mg” (Action Item)
  • Task List → “Increase Lisinopril to 10mg” (Task)
  • Task Completed → “Lisinopril increased to 10mg” (Logged to Activity Feed)

This creates a closed loop where:

  1. Information flows from encounters into tasks
  2. Tasks are tracked and completed
  3. Completions flow back into the activity feed
  4. The entire care journey is visible in one place

Nothing gets lost. Every action item becomes a task. Every completed task is logged. The entire care workflow is visible, trackable, and auditable. The Care Loop ensures that the gap between planning and execution is closed, creating a seamless flow from identification to completion.

Closing the Care Loop
